Dr. John Ditto, an ENT Specialist at Richmond Breathe Free Sinus & Allergy Center, said on Instagram that sinusitis can lead to eustachian tube blockage, causing fluid buildup and increasing the risk of ear infections.

“Sinusitis can cause inflammation in the nose,” said John Ditto, according to Instagram. “That inflammation can cause swelling at the eustachian tube. If that becomes blocked, fluid can build up behind the eardrum. Bacteria trapped in that fluid can become an infection.”

According to medical research, sinusitis-induced inflammation can affect adjacent structures in the upper respiratory tract, notably the eustachian tubes, leading to complications such as otitis media. The anatomical proximity and physiological connection between the nasal passages and the middle ear make it possible for pathological processes in the sinuses to influence ear function. A study published in the Journal of Otolaryngology – Head & Neck Surgery outlines how inflammation from sinus infections may lead to dysfunction of the eustachian tube by promoting mucosal edema and obstruction, thereby impairing normal middle ear ventilation and pressure regulation.

Once the eustachian tube becomes swollen or blocked, negative pressure may develop in the middle ear, which draws in fluid from surrounding tissues. This fluid provides a suitable medium for bacterial proliferation. Research published in The Pediatric Infectious Disease Journal supports this mechanism, highlighting that bacterial otitis media often follows viral upper respiratory infections due to resultant eustachian tube dysfunction and fluid accumulation. In particular, the presence of pathogens such as Streptococcus pneumoniae or Haemophilus influenzae in the middle ear effusion confirms the transition from sterile inflammation to active bacterial infection.

Chronic sinus inflammation and anatomical variations in the nasal and paranasal regions can contribute to Eustachian tube dysfunction, increasing the risk of developing chronic otitis media. A study published in Acta Oto-Laryngologica found that conditions such as nasal septum deviation and concha bullosa can obstruct nasal airflow, leading to impaired Eustachian tube function and subsequent middle ear diseases. These findings highlight the importance of addressing nasal and sinus pathologies to prevent recurrent middle ear infections and associated hearing impairments.

Ditto is a board-certified Otolaryngology Head and Neck surgeon and a fellow of both the American Academy of Otolaryngology–Head and Neck Surgery and the American Academy of Otolaryngic Allergy. He earned his medical degree from the University of Missouri–Columbia School of Medicine, where he was recognized with the Outstanding Achievement Award by the Student National Medical Association. Ditto completed his residency in Otolaryngology at the University of Michigan, Ann Arbor, and further specialized with a fellowship in otology at the same institution. His research during residency was published in the Archives of Otolaryngology.
